Mary's Hospital - Madison, Inpatient Pharmacy
Hourly (shift differential eligible)
-$2,000 sign on bonus for eligible hires

Full-Time: 40 hours/week (1.Monday - Friday, day and evening shift rotation (shifts between 5:00am - 9:30pm)
Every 4th weekend rotation

Assists with the medication reconciliation process in coordination with pharmacy, nursing, care management, and other healthcare professionals as appropriate.


Follows up with pharmacies, nursing homes, caregivers or others as necessary to obtain complete medication history. Promotes the hospital's retail pharmacy program through the offering of discharge prescription services if applicable. Documents, collects and compiles data, statistics and other information as required for quality assurance programs and assists with reports to provide information to facilitate leadership decision-making.
High School diploma/GED or 10 years of work experience


Frequent keyboard use/data entry.
